Due to a myriad of legal and social reasons, Oregon has one of the lowest rates of sickle cell trait and disease in the nation, yet our patients living with these inherited red blood cell conditions deserve the highest quality of care we can provide. Sickle cell trait was once thought of as a benign trait but we now know it carries health and social risks. Sickle cell disease, after decades of minimal progress, has seen significant medical progress, including gene therapy. However, social progress is slower in coming.

The Oregon Academy of Family Physicians is excited to announce the third cohort of Primary Care Beyond Clinic Walls (PCBCW). OAFP  supports better collaboration among primary care, local public health officers, and community-based organizations (CBOs) through the PCBCW program focused on community challenges. Participants will establish enduring working relationships with CBOs working on social drivers of health and local public health through mentored development of projects aimed at improving community health needs related to inequities and social drivers of  health. The Third Cohort of PCBCW will be discussing their experiences at this panel session.
